JOB SUMMARY

The Direct Support Professional is responsible for a variety of duties related to the overall care of individuals with developmental disabilities who receive Innovations Waiver Services with Premier Service of Carolina, Inc. This includes, direct care of program individuals, implementation of individual support plans (ISP), and behavior plans (BP), as appropriate. The DSP performs a variety of duties to help participants fully assimilate into their community, develop vocational skills, social skills, and meet goals outlined in the ISP–furthering their growth towards independence. The DSP is also responsible for creating a supportive environment as he or she unobtrusively assists consumers. The DSP reports to the Program Director/Regional Manager. DSPs will work the scheduled number of hours as determined by the ISP and is responsible for reviewing, comprehending, and demonstrating a strong understanding of the service(s) the DSP provides.

JOB DUTIES AND RESPONSIBILITIES

Essential Functions, include but are not limited to the following:

The Direct Support Professional: * Assists in the implementation of individual support/ behavioral plans for individuals

  • Assists individuals in the pursuit of recreational, social, and community activities based on their preferences and individual support plans
  • Assists and encourages individuals in the establishment of social networks
  • Promotes participation and involvement in community outings/events
  • Works all assigned schedules, including but not limited to, arriving and departing on time, requesting time off in advance, and notifying the supervisor of changes to assigned work schedule
  • Ensures everyone’s rights are protected
  • Treats individuals with dignity and respect
  • Observes individuals for evidence of injury or bruises and evaluates for changes in emotional and physical status
  • Reports any noted problems to the Program Director/ Regional Director immediately and completes a written incident report form
  • Maintain client confidentiality
  • Documents all program data, including but not limited to: individual service plan data, behavior plan data, community integration logs/assessments, individual funds log, mileage logs, MARs, Medical Eval form, etc
  • Documents individuals’ health (i.e. seizure charts, weight, bowel movements, fluid intake, as appropriate.)
  • Ensures that all documentation is completed accurately and in a timely manner
  • Ensures Medication Administration Record (MAR) entries are completed daily (as required)
  • Identify and develop each client’s strengths
  • Foster creativity and positive attitudes to build collaborative opportunities with other staff and clients
  • Put the primary focus on participants’ needs; person-centered awareness permeates interactions with participants, staff, and others. Participation in all activities is the choice of client
  • Create of high-quality services that result in personal growth benefits for client
  • Contribute to meeting participant needs, recognizing that he or she is vital to the growth and maintenance of the services provided

QUALIFICATIONS

  • High School Diploma or equivalent
  • Staff must meet client-specific competencies as identified by the individual’s person-centered planning team and documented in the Individualized Support Plan
  • Must have a criminal record check and a healthcare registry check, OIG as required in accordance with 10A NCAC 27G.0200
  • If providing transportation must have a North Carolina or other valid driver’s license, a safe driving record, and an acceptable level of automobile liability insurance
  • Must be capable of passing a pre-employment drug screen and TB screening

TRAINING and COMPETENCY REQUIREMENTS:

Successful completion of Training/Skills refresher training as mandated to include: * First Aid/CPR

  • Medication Administration
  • EBPI
  • Bloodborne Pathogens/ Infectious Disease
  • HIPAA/ Client Rights/ Confidentiality
  • Core Competencies
  • General Organizational Orientation Training to meet the MH/DD/SA needs of the clients as specified in the treatment/habilitation plan.
  • Other trainings as indicated

Note: Some training requirements/objectives may be satisfied by level of competency according to professional education, attainment of licensure, and continuing education. These are subject to review and approval by the state, LME, and/or Premier Service of Carolina, Inc.

Work Environment

This job involves duties that require willingness to work in a community/home setting of the person served. Work performed outside of the office may involve exposure to cigarette smoke, domestic animals/pets, household pests, uncomfortable heating/cooling, and other issues related to the domestic location in which service is delivered.
