Job Summary
- Responsible for managing the day-to-day operations of Outpatient Nutrition Therapy and Diabetes Education Programs. This includes oversight of the planning, implementation, and evaluation of the Diabetes Self-Management Education and Support (DSMES) services.
Primary Duties:
1. Manages day to day operations of the outpatient nutrition and diabetes programs.
2. Develops, implements, and evaluates policies, procedures and standards for the provision of nutrition care that reduce adverse events resulting from malnutrition.
3. Assesses, plans and evaluates compliance with best practices in patient care for outpatient diabetes services.
4. Collaborates with the Endocrinology and Diabetes Medical Director, OB Physicians, Primary Care Physicians, and the Medical Advisory Board of the outpatient education center and the multidisciplinary diabetes committee to ensure all policies and procedures necessary to care for diabetic patients are within the Diabetes Clinical Practice Guidelines- outlined by the Diabetes Team. This includes program elements outlines in our accreditation by the ADA (outpatient) and the Joint Commission (inpatient).
5. Oversees and Conducts daily patient nutritional assessment and education
6. Schedules therapy staff; dietitians, diabetes educators, diet technicians and registered nurses. Develops, coordinates and manages the monthly schedule to ensure appropriate department staffing.
7. Develops and implements a process to manage human resources, materials, and finances in coordination with the Endocrinology Medical Director.
8. Acts in a leadership role in diabetes and nutrition program activities and collaborates with other Evergreen Health service lines and physicians (Obstetrics, clinics, primary care).
9. Oversees the planning, implementation, and evaluation of the DSMES services.
10. Arranges and coordinates the activities of the Advisory Group for the Education Recognition Program (ERP) Diabetes Program.
11. Monitors, facilitates, and co-manages maintenance of DSMES team members qualifications including: CEU credits, training, competencies, licensures, and registrations.
12. Performs other duties as assigned.
License, Certification, Education or Experience:
REQUIRED for the position:
o Registered Nurse or Registered Dietitian with the American Dietetic Association
o Certified with the State of Washington as a Registered Nurse, Registered Dietitian, or Nutritionist
o 2 years of experience partnering with providers in the delivery of diabetes management
o 2 years of clinical nutrition experience in acute care, ambulatory care
o Experience in program management and development
o Assessment skills for developing a patient specific care plan.
o Must have knowledge of regulatory agency requirements as their standards apply to specific care setting, TCC, Federal, State, and TJC regulations/ requirements.
DESIRED for the position:
o Bachelor’s or advanced degree from an accredited institution with a major in dietetics, foods and nutrition.
o Experience in cross-functional development of patient care models
o Industry knowledge of diabetic DME
o Certification as a diabetes educator (CDE)
